Question: Question: My dentist wants to place four implants to hold my dentures in place. To save money, why can’t I have just two like other friends of mine had?

Answer: It is always recommended to place at least four implants to create adequate retention to secure full dentures if (and this is a big “IF”) you have an adequate amount of bone to accept the implant.

Four allows a back-up: If one implant should be rejected, you still have three left to serve as a source of good retention.

Another benefit to four is that if they are placed far enough apart on the upper jaw, the thick, bulky palate of an upper denture can be cut out, allowing you to have the luxury of feeling the roof of your mouth again with your tongue. The surface of the palate actually helps in taste sensation.

The statistics he saw on paper and the people he saw at his eight-year-old dental practice led Franklin Rios to worry about the state of Jacksonville’s teeth and gums.

About 60 percent of U.S. residents have no dental insurance, he said, and many of those who do have minimal coverage. Among his first-time patients are children who had no previous dental care and adults who delayed seeking care so long they had to have teeth pulled.

There had to be a way to provide affordable dental care, particularly preventive treatment, for those people, he told the Times-Union.

“When I started my practice … I always thought there would be something else. You get a good discount if you have insurance, but a lot of people don’t have insurance or their plan doesn’t cover many things,” he said.

Rios said he never got around to coming up with that “something else” on his own. But two other dentists did and last year he jumped on their bandwagon.

Patty Peterson considers herself someone who has “a pretty high pain threshold.”

Yet for most of her life, Peterson, 54, went to the dentist only as a last resort.

“I’d wait until something hurt, and then I’d go,” she says.

She recalls nothing in particular that led to her longtime pattern of dental avoidance, “other than for the time they tried to give me an injection in the middle of the roof of my mouth. I remember that. I really wanted to punch the dentist.”

Then, about two years ago, she became a patient of Dr. Peter Balle and is amazed at how, well, not painful her dental experiences have been since. That, she adds, is true even for such significant work as the placing of tooth implants.

“They give you drugs when you go in there. Someone has to drive you,” she says. “And you don’t really remember anything at all. You walk around and you talk to people, but you don’t remember anything.
